A report is a snapshot of live data and answers how things stand right now. A baseline records how things were back then. An assessment usually needs both - which is why the reports page says so in one sentence.
No, and that is the point. There is no update path. Deleting is possible but tied to a dedicated permission and an audit entry.
No. Work continues as normal; the state stays as it was. Optionally you can require a change request for edits to delivered requirements.
Yes. A quality gate can carry a state as its reference and optionally only pass once coverage is sufficient or all mandatory rules are met.
